Ultrasound and thoraco-abdominal computed tomography (CT) demonstrated a complex hepatic cystic lesion with a transdiaphragmatic fistulous tract trajectory, directed to the pleural space. The lesion was compatible with a complicated hydatid cyst with direct rupture to the pleural cavity.
