The patient is a 60-year-old male with a personal history of diabetes, arterial hypertension, smoking, dyslipidemia, and avascular necrosis of the hip. He presented with a one-month history of extensive, painful, and pruritic skin lesions. He was afebrile and in good general condition, with no history of international travel, contact with animals, occupational exposure to harmful substances, or unsafe sexual practices. On physical examination, denuded and crusty red-brown plaques were observed on the extensor surfaces of the forearms, arms, buttocks, thighs, and legs, with a symmetrical distribution and sparing of the trunk and face. Some lesions were ulcerated.
