A computerized tomography was performed and a solid-cystic pancreatic mass was observed. The lesion measured 10 x 8 cm with well-defined margins, a macroscopic calcification and an enhanced solid component. These findings were consistent with a mucinous cystadenoma (Fig. 1). Endoscopic ultrasonography showed a cystic neoplasm with a calcified wall and central whirlpool-shaped echogenic foci (Fig. 2). Fine needle aspiration revealed gelatinous content and cellular atypia suggestive of malignancy (Fig. 3A).
